【摘要】 为进一步规范护理人员的操作流程，保障注射安全，降低不良反应发生率，特制订《抗凝 剂皮下注射技术临床实践指南》。本指南是在《抗凝剂皮下注射护理规范专家共识》的基础上，通过系 统检索近 10 年国内外皮下注射相关最佳临床证据，聚焦抗凝剂皮下注射的操作步骤和细节，综合全国

38 所医院 46 名医护专家的 6 轮函询结果形成的。最终从注射前评估、注射前准备、注射部位及体位、皮 下注射、在院注射宣教、居家注射管理 6 个方面汇总并形成抗凝剂皮下注射技术临床实践指南推荐意见 64 条，从皮下出血、疼痛、渗（漏）液、过敏反应、弯针与断针 5 个常见皮下注射不良反应方面汇总并形成 抗凝剂皮下注射技术不良反应及处理对策临床实践指南推荐意见 23 条，旨在为临床护理人员的科学决 策提供实践依据。

Clinical Practice Guideline for Subcutaneous Injection of Anticoagulants

【Abstract 】 To further standardize the operating procedures of nurses，ensure injection safety， and reduce the incidence of adverse reactions，the Clinical Practice Guideline for Subcutaneous Injection  of Anticoagulants is formulated. The guideline is based on the Expert Consensus on Nursing Standards for  Subcutaneous Injection of Anticoagulant Agents，and is formed by systematically searching for the best evidence  on subcutaneous injection of anticoagulants in the past 10 years both domestically and internationally. The  guideline focuses on the operational steps and details of subcutaneous injection of anticoagulants，and integrates  the results of six rounds of consultations from 46 medical and nursing experts from 38 hospitals across the  country. Finally，64 pieces of recommendations for subcutaneous injection of anticoagulants were summarized  from six aspects of pre-injection evaluation，pre-injection preparation，injection site and position，subcutaneous  injection，hospital injection education，and home injection management. 23 pieces of recommendations for  adverse reactions and strategies of subcutaneous anticoagulant injection from five common adverse reactions of  subcutaneous injection，including subcutaneous bleeding，pain，exudation（leakage），allergic reactions，curved  needles and broken needles，aiming to provide practical basis for scientific decision-making of clinical nurses.

静脉血栓栓塞症（venous thromboembolism，VTE） 包括深静脉血栓（deep venous thrombosis，DVT）和肺   血栓栓塞症（pulmonary thromboembolism，PTE）。抗   凝治疗是 VTE 防治的基础［1-3］。除此之外，抗凝治   疗也被广泛用于心血管科、骨科、妇产科、肾内科等   相关疾病治疗［3］。目前临床上可供皮下注射的抗  凝剂包括低分子肝素类、磺达肝癸钠。中国静脉介   入联盟、中国医师协会介入医师分会外周血管介入   专业委员会于 2019 年发布了《抗凝剂皮下注射护理   规范专家共识》，旨在规范化、标准化抗凝剂的安全   注射。该共识撰写期间临床抗凝剂普遍为预灌式针   剂， 历时 4 年，在抗凝剂剂型和配套注射器不断更新   的背景下，聚焦皮下注射操作技术的证据逐渐增加。 为进一步规范临床护理人员的皮下注射技术操作   流程，通过系统的文献检索，提炼并汇总抗凝剂（含   冻干粉）皮下注射技术操作流程，根据最佳证据、临   床实践和情境、患者意愿及专业人员判断，最终形   成《抗凝剂皮下注射技术临床实践指南》，下文简称   《指南》。
